1. About the Foundation

Bandari Charitable Foundation is a registered charitable trust operating under the laws of India, with its principal office at Mancherial, Telangana – 504208. The Foundation is:

• Registered under the Societies Registration Act / Trust Act (Telangana)
• Registered under Section 12A of the Income Tax Act, 1961 (exemption from income tax)
• Holder of 80G certification (Form 10AC) — donations are tax-deductible for donors
• Registered on the NGO Darpan portal of NITI Aayog, Government of India
• Holds a valid PAN (AAJCB1636M) and files annual ITR

By accessing www.bandari.co.in or using any of our services, you agree to be bound by these Terms & Conditions.

2. Membership Cards — Health Card Programme

2.1 Card Types and Pricing

The Foundation offers the following Health Membership Cards valid at Thirupathi Hospitals, Mancherial:

• Silver Family Health Card — Launch Offer: ₹1,000 per year (Regular Price: ₹2,000) | Benefits: 20% OFF OP Consultations, Lab Tests, and Imaging Services
• Gold Family Health Card — Launch Offer: ₹3,000 per year (Regular Price: ₹6,000) | Benefits: All Silver Card benefits + 20% OFF IP Consultations, General Ward, ICU, and 10% OFF Pharmacy
• Diamond Family Health Card — Launch Offer: ₹5,000 per year (Regular Price: ₹10,000) | Benefits: All Gold Card benefits + 20% OFF VIP & Special Rooms, Selected Surgeries, and 20% OFF Pharmacy
• Platinum Family Health Card — Launch Offer: ₹10,000 per year (Regular Price: ₹20,000) | Benefits: All Diamond Card benefits + 100% FREE OP Consultations, 20% OFF IP Procedures, Priority Admission, and 20% OFF Pharmacy
• Corporate Family Health Card — Launch Offer: ₹2,500 per year (Regular Price: ₹5,000) | Benefits: 20% OFF IP Consultations, General Ward & ICU Bed Charges, and 10% OFF Pharmacy

Card prices are subject to revision by the Foundation. Prices at time of purchase are final for the purchased membership period.

2.2 Eligible Services (Discount Applicable On)

• OP Consultation (All Doctors)
• Specialist Consultation
• Laboratory Tests
• CT Scan, X-Ray, Ultrasound, ECG
• Pharmacy (All Medicines)
• Inpatient (IP) Bills (Gold/Diamond/Platinum only)
• Surgeries (Diamond/Platinum only)

2.3 Exclusions (Discount NOT Applicable)

• Implants and Stents
• Medicines billed at MRP (where restricted by law)
• Blood and Blood Products
• Vaccines
• Outsourced Investigations
• Insurance / Aarogya Sri / TPA Packages
• Government Taxes
• Any service under special promotional offers
• ICU Charges and OT Procedure Charges (Silver/Gold)

2.4 Card Validity and Activation

• Membership cards are valid for 12 months from the date of payment
• Cards are activated within 24 hours of successful payment
• Digital card details are available in your Member Dashboard
• Cards are non-transferable and for personal use only (family members may be added as per plan)

2.5 Corporate Cards

• Corporate Family Health Cards are available for registered company employees
• Corporate memberships are priced at a flat rate of ₹2,500 per year per family card under the launch offer (Regular Price: ₹5,000)
• Provides comprehensive coverage for the employee and up to 4 family members

3. Donations

Donations made to Bandari Charitable Foundation:

• Are voluntary contributions to support our healthcare, education and empowerment programs
• Are eligible for income tax deduction under Section 80G of the Income Tax Act, 1961
• Will be acknowledged via email receipt within 3 working days
• Minimum online donation: ₹100
• Donations are non-refundable once processed (see Refund Policy)

4. Payment Terms

• All payments are processed in Indian Rupees (INR)
• Payments are accepted via Credit Card, Debit Card, UPI, Net Banking, and Wallets through our payment gateway partner (Razorpay)
• Payment gateway charges (if any) are borne by the Foundation and not passed to the user
• Your payment data is handled by PCI-DSS certified systems — we do not store card numbers or CVV
• Transactions are secured via SSL/TLS encryption

5. User Account and Responsibilities

• You are responsible for maintaining the confidentiality of your OTP and account credentials
• You must provide accurate and complete information at the time of registration
• Multiple accounts for the same mobile number are not permitted
• The Foundation reserves the right to suspend or terminate accounts that violate these terms
• You must be 18 years or older to create an account

6. API Integration with Thirupathi Hospitals

Upon successful membership card purchase, your card information is transmitted via a secure API to Thirupathi Hospitals to enable real-time discount application at point of service. By purchasing a membership card, you consent to this data sharing.

7. Disclaimers

The Foundation reserves the right to modify, suspend, or discontinue the Health Card Programme at any time with 30 days' prior notice to active members.

8. Intellectual Property

All content on this website — including text, graphics, logos, and software — is the property of Bandari Charitable Foundation and may not be reproduced without prior written permission.

9. Governing Law and Dispute Resolution

These Terms & Conditions are governed by the laws of India. Any disputes shall be subject to the exclusive jurisdiction of courts in Mancherial, Telangana, India.

10. Changes to These Terms

The Foundation reserves the right to amend these Terms at any time. Updated terms will be posted on this page with a revised effective date. Continued use of our services constitutes acceptance of the revised terms.
